Privacy Policy is an on-line registry service, designed to assist people who need a family doctor. This service is managed by South Calgary Primary Care Network (SCPCN) on behalf of other Primary Care Networks (PCNs) in the Calgary area, in collaboration with Health Link and Alberta Health Services.

This statement of privacy applies to the web site and governs data collection, usage and disclosure. By using this website, you agree to the data practices described in this statement as well as the Terms of Use. SCPCN upholds the confidentiality and privacy of individual’s personal and health information while collecting, using and disclosing information in compliance with the Personal Information Protection Act (PIPA) and the Health Information Act (HIA).

SCPCN will not collect, use or disclose personal or health information via the website in any manner that is not in accordance with FOIPP, HIA, or its established policies.

The personal information you voluntarily provide on the web registry form is collected, used and disclosed to help you find a family doctor and to plan health programs relevant to you and your community, in accordance with provisions of Alberta’s Health Information Act.

Any personal information we receive from you through this site will be handled and stored in a secure manner to protect it from unauthorized access or disclosure.

IP address, browser type, domain names, access times and referring Web addresses) is automatically collected by SCPCN when you access this website.
